What materials need to be signed and returned to complete registration?

Team Managers will distribute and collect all other necessary forms after teams are formed. Do NOT mail in any forms to the PO box!
How do I use my credit balance from last year to pay my registration assessments?
At this point our accounting system from last year is not integrated with our new website. So to use your credit balance from last year please follow these steps:
1) Register your player and choose 'Pay Offline'.
2) To pay by credit card, return to the website and click on 'Online Payments'
3) Calculate the amount you owe, deduct your credit balance, and pay that amount.
4) To pay by check, mail your check for the reduced amount to CCHA, PO Box 1006, Chanhassen, MN 55317.
5) If you do not know your credit balance, email accountant@cchockey.org before beginning your registration to receive a statement
What level is my player eligible for?
The 2011-12 Youth Age Classifications are:
Bantams 7/1/96 - 6/30/98 or in at least 8th grade
PeeWees 7/1/96 – 6/30/00 or in at least 6th grade
Squirts 7/1/00 – 6/30/02 or in at least 4th grade
Mite 7/1/02 – 6/30/04 or in at least 2nd grade
Mini-Mites 7/1/04 – 6/30/05 or in at least 1st grade
Termites 7/1/05 – 6/30/06 or in at least Kindergarten
The 2011-12 Girls Age Classifications are:
14U 7/1/96 – 6/30/98 or in at least 8th grade
12U 7/1/99 – 6/30/00 or in at least 6th grade
10U 7/1/00 – 6/30/02 or in at least 4th grade
8U 7/1/02 – 6/30/04 or in at least 2nd grade
6U 7/1/04 – 6/30/06 or in at least Kindergarten
