Checkbook Instructions
Before CCHA can provide the checkbook to you, we need to update you to be an authorized signer on the checking account. If you were a team treasurer in past years, this process still needs to be completed as the bank removes signers at the end of each hockey season. Due to the timing between team formation and the first scrimmages and team events it is normal for someone on the team (treasurer, manager, etc.) to fund the initial activities (e.g. referee payments for scrimmages). These expenditures should be tracked and reimbursed by the team upon funding the account.
- Please e-mail the CCHA Treasurer (treasurer@cchockey.org) your home address, cell phone number, and preferred e-mail address to be used for communications and recordkeeping.
- Complete the Authorized Signer form below. This is required to be completed and electronically submitted directly to Old National Bank. The bank will need BOTH the completed form and a copy of your current driver's license to complete the paperwork on their end. Please send the completed form to Diem Tran; Diem.Tran@oldnational.com.
Please note: The team CCHA business checking account CANNOT be linked to a Venmo account. Zelle is available along with online banking access to all checking accounts. Select Online Banking Access on the attached form.
Once you receive authorization from the bank, please reach out to Steve Japuntich, the CCHA Treasurer, via text/phone call (763-742-2829) to arrange a time to pick up your team checkbook from him.
